How can we transfer into your practice?
- You will need to fill out an authorization for records form. This authorizes us to request a copy of your past medical from your past medical records from your previous pediatrician so we can have good continuity of care.

Where can I get the authorization for record form?
- This can be done either at our office or your present pediatrician's office. The form is available on our website under Forms as well.

How soon can be schedule a well visit appointment for a new patient?
- We can schedule your well visit appointment as soon as your previous medical records are received by our office. Medical records from your previous pediatrician can differ in how long they take to get to us. Some records from doctor offices can take 1-2 weeks, from Hospital or larger facilities 1-2 months, and longer for out of state transfers.

How soon for a sick visit for a new patient?
- We try to see all children with urgent care emergencies the same day of the call. Calls early in the day are scheduled as soon as possible. If the calls are later in the day, we can not guarantee a visit that day; most are scheduled for the first appointments of the next day.

What if I call and you are closed?
- We are always available to take your calls. If we are away from the office, we have a 24 hours answering service. They will contact the on-call doctor for any emergencies. You are always welcome to leave us a detailed message. We will return your call as soon as we are able.
